New Patient Registration

Eligibility Requirements

The Primary Care Clinic at the Urban Inter-Tribal Center of Texas serves enrolled members of federally-recognized American Indian tribes and Alaska Natives.

Adult New Patient Appointment

To establish a chart at the Urban Inter-Tribal Center of Texas you must first schedule an appointment for a New Patient Physical. There is currently a 10 to 12 week wait time for a New Patient appointment. To be placed on the New Patient wait list, please call (214)941-1050 EXT. 200.

Download and print the New Patient Registration Packet before your appointment or pick up a registration packet at the UITCT Primary Care Clinic, 1261 Record Crossing Rd.

New Patient Packet – ADULT

Required Documents – Adults
– Certificate of Degree of Indian Blood (CDIB) Card or Tribal Enrollment Document
– Driver’s License or State ID Card
– Health Insurance Cards (Insurance information must be provided for those with existing coverage)
– List of current medications

Bring your completed New Patient Registration Packet with all the required documents to the Clinic Front Desk 30 minutes before your New Patient Physical appointment. New Patient Appointments must be kept before additional services can be rendered.

Child New Patient Appointment

Parents must come in to the Primary Care Clinic to register their child. Anyone that is not a parent must provide legal documentation that they have custody, guardianship or foster care. Bring in a completed New Patient Registration Packet – CHILD and required documents.

New Patient Packet – CHILD

Required Documents – Children
– Certificate of Degree of Indian Blood (CDIB) Card or Tribal Enrollment Document (May use parent’s CDIB card until their 18th birthday)
– Certified birth certificate
– Social Security Card
– Immunization records
– Parent’s Driver’s License or State ID Card
– Health Insurance Cards (Insurance information must be provided for those with existing coverage)
– List of current medications (prescription and over-the-counter)

There is not a wait list for children’s new patient appointments. The registration clerk will enter the information and your child will then be able to access services. If you are registering your child on the day of their appointment please arrive at the Clinic 30 minutes prior to their appointment time.
